Plaintiff, CACH, LLC, by its attorney Harrison Ross Byck, by way of complaint
against defendant(s) SONJA SCHULTHEIS, avers the following:
Plaintiff, CACH, LLC, is a Colorado limited liability company doing business at 4340
S. Monaco Street- 2nd Floor; Denver, Colorado 80237.
2. Defendant, SONJA SCHULTHEIS, is an individuals residing at 20 W GREEN
STREET; SHIREMANSTOWN, PA 17011.
3. Defendant, SONJA SCHULTHEIS, is indebted to CHASE MANHATTAN BANK on
an account stated by and between them in the amount of $5,457.61 which balance
was due and unpaid as of January 26, 2005 for credit card account number 4029 3600
1102 5050. <Exhibit A>
4. On or about July 5, 2005, Chase Manhattan Bank sold the debt for good and valuable
consideration to plaintiff, CACH, LLC. <Exhibit B>
5. Defendant (s) SONJA SCHULTHEIS last tendered a payment on this account on or
about February 15, 2007 for $150.00.
6. A copy of the credit card agreement is attached hereto. <Exhibit C>
7. Plaintiff is entitled to charge-off account finance charges of $ -0-. <Exhibit A>
8. Plaintiff is entitled to pre-litigation charge-off interest of $0.15 per day from the
default date (1.00% annual percentage rate x $5,457.61/ 365 days) or $0.15 x 392 days
_ $58.61, which is accrued interest through the date of filing, plus an award of late
fees of $-0-, plus court costs $178.50 and reasonable attorneys fees of $600.00.

9. The defendant, being indebted to the plaintiff in the sum or $6,294.72 upon the
account stated by and between them did promise to pay said sums upon demand.
Demand has been made for payment of $6,294.72 and the defendant has failed to
remit payment.
WHEREFORE, plaintiff demands judgment against the defendant for $6,294.72 together with
other interest, costs of suit, and an award of reasonable attorney's fees.

1. I am authorized on behalf of Chase Bank USA, N.A. to make this affidavit.
2. SONJA SCHULTHEIS had a credit card account with Chase Bank USA, N.A.,
account number 4029360011025050. The account was sold and transferred to
CACH, LLC, on or just prior to 6/24/2005.
3. At the time of the sale to CACH, LLC, the amount pursuant to the terms of the
cardholder agreement between Chase Bank USA, N.A. and SONJA
SCHULTHEIS was $9,157.61.
4. Your deponent states that to the best of deponent's knowledge, information and
belief that there was no unaccredited payment, just counterclaims or offsets
against the account when it was sold.
5. Your deponent acknowledges that in making this affidavit that CACH, LLC is
now the owner of said account, and authorized to collect, settle, adjust,
compromise and satisfy the same and that Chase Bank USA, N.A. has no further
interest in said account for any purpose.
