Petitioner is an adult individual residing at 5809 Stephens Crossing, Mechanicsburg, Cumberland County, Pennsylvania 17050. 2. Petitioner was born Laxman Kapur on March 2, 1971. 3. Various family members of Petitioner, including Petitioner's father, have the name Mohun or Mohan as their first or middle name. 4. Petitioner is most commonly known as and referred to by his family, friends, and co- workers as Mohun Kapur. 5. Petitioner is now desirous of legally assuming the name he is most commonly known by. 6. Petitioner has resided at the following addresses during the five years preceding the filing of this Petition: October 30, 2006 to Present 5809 Stephens Crossing Mechanicsburg, Pennsylvania 17050 April 30, 2003 to October 30, 2006 May 2002 to April 30, 2003 4090 Caissons Court Enola, Pennsylvania17025 684 St. John's Drive Camp Hill, Pennsylvania 17011 7. There are no judgments or decrees of like character of record against the Petitioner.
