The Plaintiff is a corporation duly licensed to engage in the casualty insurance business in the Commonwealth of Pennsylvania, and having an office located at P.O. Box 41, Concordville, Pennsylvania 19339. 2. The Defendant is an adult individual residing at 32 Adams Street, Enola, Pennsylvania 17025. 3. On or about April 9, 2003, Plaintiff insured a vehicle owned by Robert J. Pusti, which vehicle sustained damages totaling ONE THOUSAND THREE HUNDRED "TWENTY-ONE and 36/100 ($1,321.36) DOLLARS as a result of the negligence of the Defendant. 4. Defendant entered into an installment agreement to repay to Plaintiff the amount of money damages caused by the negligence of the Defendant. A copy of said installment agreement is attached hereto marked as Exhibit "A" and incorporated herein by reference. 5. Defendant after entering into the agreement on May 13, 2003 has paid a total of $200.00 and therefore is in breach of the installment agreement.
