NOW, this 2'1 \ day of April, 2008, pursuant to the Request of Petitioner,
Church of God Home, Inc., the facility providing residential services to Bertha Stone, to
be appointed Court Authorized Representative;
IT IS ORDERED, that said Request is granted and, effective upon the filing of
his written consent to so serve, Shaun O'Toole, Esquire, is appointed pursuant to 55 Pa.
Code 8163.3, as the Court Authorized Representative of Bertha Stone, and in such
capacity is authorized to pursue medical assistance benefits for Bertha Stone, and act in
her stead regarding all matters regarding public assistance.
This ORDER and the appointment made pursuant thereto shall supersede any
existing powers of attorney executed prior to the date of this Order.
