Mateya, Esquire, and in support avers the following:
1. Petitioner herein is Cindy F. Bunner, an adult individual presently residing at 389
Meadow Trail, Mechanicsburg, Cumberland County, Pennsylvania.
2. All legal papers may be served on Petitioner by service upon her counsel, Mark A.
Mateya, Esquire, with a mailing address of P.O. Box 127, Boiling Springs, Pennsylvania 17007.
3. Petitioner was born on November 7, 1955, to Joseph and Anna Masternak and was
named Cynthia Faith Masternak. A copy of the Certificate of Birth No. 2348650-55 evidencing
Petitioner's correct surname is attached hereto as Exhibit "A" and is incorporated herein by
reference.
4. Petitioner and her husband, Alfred L. Padilla were issued Marriage License No. 608
on July 10,2006 by Glenda Farner Strasbaugh, Clerk of the Orphans' Court of Cumberland
County, Pennsylvania. A copy of the Certification of Excerpts from the Marriage License
Record is attached hereto as Exhibit "B" and is incorporated herein by reference.
5. Upon review of the aforementioned Certification of Excerpts from the Marriage
License Record, Petitioner discovered that her parents last names were incorrectly listed as
Joseph Masterson and Anna Masterson.
6. The correct name for Petitioner's parents should be Joseph Mastenrak and Anna

WHEREFORE, Petitioner respectfully requests that this Honorable Court issue a
corrected Certification of Excerpts From Marriage License Record and/or Marriage License
correctly listing Cindy F. Bunner's parents as being Joseph Masternak and Anna Masternak.
