AMENDED PETITION
FOR COURT APPROVAL TO PAY PREPAID BURIAL
TO THE HONORABLE JUDGES OF SAID COURT:
AND 1VOW, this 8~ day of May 2008 comes the guardian, Constance
E. Stoneroad, of Keystone Guardianship Services, of the above-
captioned incapacitated individual, petitions as follows:
1. Claire R. Weldon is presently a resident of the Sarah
A. Todd Memorial Home, located at 1000 West South Street,
Carlisle, Cumberland County, Pennsylvania.
2. Claire R. Weldon is receiving a monthly social
security payment paid directly to the representative payee,
Sarah A. Todd Memorial Home in the amount of $545.00 toward her
care and housing.
3. Keystone Guardianship Services was appointed Guardian
for Claire R. Weldon, an alleged incapacitated person by the
Cumberland Country Orphans Court by order dated August 6, 2007.
4. The expenses due Sarah Todd Memorial Nursing Home for
the care of Claire E. .Weldon have surpassed the amount of the
proceeds of the pending sale of the Weldon Cafe business and real
estate.

5. Keystone Guardianship Services has filed a claim with
Department of Welfare for Medical Assistance for Claire Weldon
to pay for her nursing home expenses.
6. There is no burial reserve set up for the funeral
expenses of Claire R. Weldon.
7. There is a life insurance policy held with ING
ReliaStar Life Insurance Company with a cash surrender value of
$3,415.10 as of April 19, 2007.
8. The ING Relia3tar Life Insurance Policy has a face
value of $10,000.00
9. Some of the monthly premiums paid to ING to keep the
life insurance policy from lapsing were paid by Claire Weldon's
daughter, Janice Young and her daughter's partner, Mike Gingrich.
Recently the premiums have been paid from some earnings of the
Weldon Cafe.
10. There will be no money available after the payment of
Mrs. Weldon's nursing home expenses to continue to pay the
insurance premiums for the life insurance coverage.
11. The Weldon Cafe real estate and the Liquor License and
equipment held under Goldring & Lipman, Inc. are under contract
to be sold. All outstanding issues relative to the sale have
been addressed. The transfer of the Liquor License is in process
and upon the approval of the transfer the settlement will be
completed.
12. The balance of cash needed to purchase the burial
agreement for Claire Weldon will be taken from the proceeds of
the sale prior to paying the Sarah Todd Memorial Nursing Home.

13. Funeral arrangements have been completed with the
Myers Funeral Home, Tnc., 37 Main Street, Mechanicsburg,
Pennsylvania 17055. (see copy of Statement of Funeral Goods and
Services and Forethought Life Insurance Company contract
attached as Exh.ibi t "I ") .
WHEREFORE, the aforementioned guardian respectfully requests
this Honorable Court enter an Order directing the following:
1. Keystone Guardianship Services authorization to pay
$4,995.00 to Forethought Life Insurance Company for
the prepaid burial arrangements for Claire R. Weldon
from the proceeds of the ING Life Insurance Policy and
the difference from the proceeds of the sale of her
real estate and the Liquor License held under
