On Friday, November 17, 2006 Judge granted to the Cumberland County`~Office of-_- ~
Aging a Temporary Emergency Involuntary Intervention court order for Elizabeth Floyd.
The order was necessary because client's caregiver died, she was being evicted from
the house and she was unable to care for herself due to dementia. She was
transported to the Holy Spirit Hospital's emergency room far medical evaluation. Upon
discharge she was admitted to dementia unit of The Todd Home, Carlisle, PA.
At the time of her admission, Ms. Floyd was diagnosed with a urinary tract infection,
uncontrolled hypertension and dementia with delusions. On May 15, 2007 Judge Ebert
appointed the Cumberland County Office of Aging Permanent Plenary Guardian of
Person and Estate for Elizabeth Floyd.
Since that time she has also been diagnosed with ambulation dysfunction, antropthy, a
history of a vertebrae compression, macular degeneration, osteoperosis,
kyphoscoliosis. She was hospitalized once during the year for a fainting episode. Client
has adjusted well to her new surroundings. The only problem that has occurred was that
Elizabeth liked to order the other residents around and yell at them if they did something
wrong. We tried to explain to her that those residents did not realize what they were
doing but she did not believe this. This behavior has decreased with time. She is very
pleased with the food at the home. She attends and participates in multiple group
activities. Her current medications include actonel 35mg weekly, hydrochlorothiazide
12.5mg QD, K-tabs 10MEQ TID, lidoderm patch 4% 12 hr on 12 off, metoprolol tartrate
25mg '/2 tab QD, neurontin 100mg, every 12hours, oscal oyster shell calcium 500 mg
BID, vicodin 5-500mg 4hr prn, vicodin 5-500mg 3 x day and naprosyn 375mg BID for
pain. They do a weekly skin assessment. She wears a watchmate on her right ankle.
It is the request of the Cumberland County Office of Aging that the agency retain
Guardianship of Person for Elizabeth Floyd. The Todd Home has demonstrated an
adequate quality of care for her medical and mental problems. We believe, therefore,
that Ms. Floyd should continue to reside in that nursing facility.

On May 15, 2007 Judge Ebert appointed the Cumberland County Office of Aging
Permanent Plenary Guardian of Person and Estate for Elizabeth Floyd.
Upon appointment the Agency filed a change of address with the post office and
requested Commerce Bank change her account to a guardianship account. Apparently
they did not do this correctly and client's social security checked was not directly
deposited. We then opened a guardianship account at M&T and transferred her funds
into that account.
Ms. Floyd's current income from Social Security if $1569. The monthly payments to The
Todd Home are currently $1524. In addition, there were some payments made for
personal items, supplies, and clothing and medical expenses not covered by medical
assistance. We met with a funeral home to get some information concerning burial
arrangements but client did not have sufficient funds to do that at the time. Since
Elizabeth is on Medical Assistance, she can only retain $45.00 of her income with the
remainder going to the Home. The balance in her account on May 3 2008 was 3871.42
with her social security just received. We anticipate that she will have sufficient funds to
make preliminary funeral arrangements in the next few months.
Attached is a financial summary of Elizabeth Floyd's income and expenses. Also
attached is a spread sheet representing her checking account register.
