PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
The Petition of Brandon Lee Potteiger, by his attorney, Ruby D. Weeks, Esquire,
respectfully represents:
1. Brandon Lee Potteiger, an adult individual, resides at 40 Harmony Hall Drive,
Carlisle, Pennsylvania and is herein called Petitioner.
2. (a) Ricky Lynn Potteiger, is an adult individual whose current address is 45 West
Main Street, New Kingston 17050. He is the adoptive father of the subject of this Petition. He
was never married to the child's natural mother.
(b.) Kathleen R. Gelinas-Rice, is an adult individual whose current address is 40
Harmony Hall Drive, Carlisle, Pennsylvania 17013. She is the natural mother of the subject of
this Petition.
3. Petitioner desires to change is name of Brandon Lee Potteiger to Brandon Lee Rice,
the surname of his natural father.
4. Petitioner's reasons for desiring this name change are:
(a.) To have his surname correspond to his natural mother's sumame, Kathleen
Gelinas-Rice.
(b.) To have the surname of his birth father.
(c) Therefore, Petitioner seeks Court approval of the desired name change so
that said name change will be complete and final.
5. During the five (5) years preceding the filing of this Petition up to and including the
date of this Petition, Petitioner has resided:
(a) 40 Harmony Hall Drive, Carlisle, Pennsylvania, from January 2002 until
present.
(b) 1345 Vickroy St., Pittsburgh, PA 15219, from 2002 until 2003 (attending
college)
(c) 219 Front Street, Box 100, Boiling Springs, PA 17007, from 1999 - 2002
(d) 14 E. Eager Street, Apt. 2A, Baltimore, MD 21202, from 1999 - 2002
WHEREFORE, Petitioner prays Your Honorable Court, pursuant to the Act of April 18,
1923, PL 75, § 2 as amended, 54 Pa. C.S.A. §§ 101-102, 701-705 (Purdon Supp. 1985):
(a) To fix a heating on this Petition, and
(b) To direct that notice be given of filing of this Petition and the heating.

father of the subject of this Petition.

~l~pon consideration of the attached Petition and after hearing, it is found as a fact that the
requirements of the Act of April 18, 1923, PL 75, as amended, 54 Pa. C.S.A. § 101 - 102, 701 -
705 (Purdon Supp. 1985), have been satisfied and the name of Brandon Lee Potteiger is changed
to Brandon Lee Rice.
