Satisfactory proof having been made, and good cause appearing, IT IS ORDERED that: The Certificate of Death records of the Bureau of Vital Statistics, Department of Health, Common Wealth of Pennsylvania dated November 30, 1934, File No. 100413, Registered No. 114, be changed to reflect her true and correct name as it appears on her Birth Certificate, which is Norma Sedgwick. 2. The correct name of her father shall be corrected to Roland B. Sedgwick, as it appears on the child's Birth Certificate. 3. The correct name of her mother shall be corrected to Mary Isabel Hayford as it appears on the child's Birth Certificate. Birth Date and Place: The minor was bom on January 11, 1922 in Salt Lake City, Salt Lake County, State of Utah A certified copy of the minor's birth certificate is attached to this petition. 2, Birth Name: The minor was named Norma Sedgwick at birth, and as of the date of this petition, the minor has always been known by that name, with the following exceptions, if any: Death Certificate Bureau of Vital Statistics Depa~hnent of Health Commonwealth of Pennsylvania Regi~hafion District No. 21-06-82 File No. 100413 Registered No. 114 Dated: November 30, 1934 3. Residence: The minor is DEAD The Petitioner resides at 3143 West 7300 South, Spanish Fork, UT 84660, and has lived at this address for 24 years. 4. Minor's Parent~: The father of the minor is named Roland Bumingham Sedgwick, and he is DEAD. The mother of the minor is named Mary Isabelle (Isabel) Hayford, and she is DEAD. 5. Reason for Name Change: Petitioner seeks to change said minor's name to Norma Sedgwick and offers the following reasons for seeking to do so: Norma and her two half sisters were murdered in 1934 in the woods near Carlisle. Her correct name was not known at that time. State Police office Harrisburg Barracks, mistakenly identified her as Norma Noaks. Her mother and true father were not known at that time. 6. Legal and Creditor Status: The minor does not have any outstanding judgments, has never been convicted of a crime, and is not involved in any pending legal actions. None 7. Additional Declarations: In addition to the foregoing statements, the Petitioner further declares: 1. Corrections to Norma's correct name were printed in many major newspapers, including: Salt Lake Tribune, dated December 1, 1934 The Evening News, Harrisburg, PA, dated December 1, 1934 Note: Similar articles appeared in other newspapers, both on that date and subsequently in written accounts of the '~Babes in the Woods" murders. See Attachment" B" 2. Her father, Roland Burningham Sedgwick, sought custody of Norma, on July 21, 1932, after her mother's death, but her step-father lett the jurisdiction of Utah courts before a restraining order could be served.
