WITH COURT ORDER
The undersigned, Charles E. Schmidt, Jr., who, being duly sworn
according to law deposes and states that:
1. The Honorable Kevin A. Hess entered an Order on February 7,
2008, directing that settlement funds in the amount of $5,361.49 be deposited
in compliance with Pa. R.C.P. 2039(b)(2), by Justin R. Self, parent and natural
guardian for Aidan R. Self, a minor, in Certificates of Deposit with the

remaining balance, if any, in a savings account, insured by a federal
governmental agency, such account not to exceed the amount to which
accounts are insured by FDIC.
Each Certificate of Deposit and account shall be marked as follows:
"This money shall be held in trust, not to be redeemed,
except by Order of the Court before May 5,2023."
2. In accordance with the terms of said Order, the undersigned did,
on May 13, 2008, set up the requested account, and proof of same is attached
hereto.

Order
AND NOW, this 7" day of February] 2008, pursuant to Pa. R.C.P.
2039 and 2206] and upon consideration of the Petition for Approval of Minor's
Settlement for Wrongful Death Action, it is hereby ORDERED that the
Petitioner is authorized to enter into a settlement in the gross sum of $15,000.
Petitioner is authorized to sign the Release attached to the Petition as Exhibit
t:'tJ."
The settlement proceeds shall be distributed as follows:
TO: Schmidt Kramer PC, $2,500.00 as attorneys' fees (1/3 or $835, of
\vhich, is to be paid to Michael Bangs, Esquire as a referral fee).
TO: Schmidt Kramer PC, $2,138.51 as costs.
The balance of the settlement is apportioned as follows:
Wrongful Death Action:
TO: Kathleen A. Smith (for final expenses) $5,000
TO: Aidan R. Self, minor son of Kathleen Ann Conn, $5,361.49.
Said sums payable to the minor shall be invested by Justin R. Self,
parent and natural guardian, for the minor. Said sum shall be deposited in
compliance "vith Pa. R.C.P. 2039(b)(2), by Justin R. Self, parent and natural
guardian.
Each account shall be marked as follows:
"This money shall be held in trust, not to be redeemed,
except by Order of this Court, before May 5, 2023."
The law firm of Schmidt Kramer PC shall oversee the execution of the
preceding paragraph.
