RECEIPT AND RELEASE
I, the undersigned hereto, as a named legatee in the Will of Carl R. Burrell, desire
that his Estate be distributed without the formality of a Court accounting.
I acknowledge that I have carefully examined the accounting for the Estate of Carl
R. Burrell as prepared and verified by Lawrence J. Neary, Esquire, Administrator C.T.A.,
and find it to be true and correct and that I am not aware of any outstanding or unsatisfied
claims against the Estate.
I acknowledge that I have received and am in agreement with distribution of the
Estate Assets that has been proposed on Schedule C, including specifically, the purchase
of a 15 year annuity with quarterly payments.
I hereby release and discharge the Estate of Carl R. Burrell and Lawrence J. Neary,
Esquire, Administrator C.T.A., from any and all actions, liabilities, claims and demands in
connection with the administration and distribution of the assets of the Estate.
In witness whereof, and with the intent to be legally bound, the undersigned has set
his hand and seal.
