Niebel, an adult individual residing at 2923 Maple Road, Camp Hill, Cumberland County, Pennsylvania 17011. 2) Petitioner is the natural mother and custodian of a minor child, Makoa Joseph Niebel Allen (DOB: September 2, 2002) who also resides at 2923 Maple Road, Camp Hill, Cumberland County, Pennsylvania 17011. 3) It is the intention of Petitioner to obtain a name change for her child. She simply desires her surname be second behind the child's natural father's surname. Therefore, the child's name would change from Makoa Joseph Niebel Allen to Makoa Joseph Allen Niebel. 4) Petitioner requests the change of surnames as it will be beneficial for Petitioner and the child in their dealings with the child's school, friends and community. The use of the last name will be less confusing, allow for consistency in the household and be more comfortable for the child as he starts school. The child's natural father lives in Texas and has had minimal contact with the child since his birth. Further, Father is not included in the child's community on a daily basis. 5) During the last five years, Petitioner and the child have resided at the following addresses: DATES ADDRESSES 6/2003-2/2004 2110 Mayfred Lane Camp Hill, PA 2/2004-8/2006 29 N. 23rd Street Camp Hill, PA 8/2006-4/2008 405 Haldeman Blvd. New Cumberland, PA 4/2008-Present 2923 Maple Road Camp Hill, PA 6) The child's natural father is James E. Allen, III, an adult individual whose last known address is 4332 Riverside Drive, Dallas, Texas 75241. 7) The natural Father is not joining in this petition but notice of same shall be made to him. 2 WHEREFORE, Petitioner requests the name of the minor child, Makoa Joseph Niebel Allen, be changed to Makoa Joseph Allen Niebel.
