On May 19,2005, Permanent Guardianship of Person for Mary C. Dugan was granted by
Judge Kevin Hess to Cumberland County Office of Aging. Prior to this appointment Mrs. Dugan
resided in her own home at 308 Mt. Allen Rd. Mechanicsburg, Pennsylvania. She was admitted
to The Bridges at Bent Creek in Mechanicsburg on April 26, 2005 after the outcome of an
Emergency Guardianship hearing. The Bridges at Bent Creek is an assistive living facility. At the
time of the Permanent Guardianship hearing Brian Brooks of the P A Guardianship Association
was appointed Permanent Guardian of Estate. Judge Hess on November 28, 2005 approved the
withdrawal of P A Guardianship Association and appointed Keystone Guardianship Services as
Permanent Guardian of the Estate of Mary Dugan.
Mrs. Dugan continues to reside in the dementia unit at The Bridges. Her primary medical
conditions of depression, hypertension, arthritis and GERD have remained stable. Mrs. Dugan
was being seen by University Physicians which was located a mile from her residence. The
Office of Aging would provide the transportation and be with her during her visit. Due to Mrs.
Dugan's high anxiety and being disoriented when out it was decided to have MD at Home visit
her at the Bridges. The doctor comes to the facility on a regular basis and this has worked out
very well. During the past year Mrs. Dugan experienced fainting spells that resembled seizures
and she was in the Hershey Medical Center for evaluation. She also was seen locally by a
neurologist and cardiologist. It was determined that she did not have seizure activity. She also
had oral surgery for the removal of two broken teeth. Her cognitive status continues to decline
and she at times doe not remember those who see her on a regular basis. Also due to her
cognitive impairment and high anxiety it was decided that The Bridges would transport Mrs.
Dugan for any out of the facility visits. A companion would ride with her and be with her until
she returned. I was also present during all her medical visits. The staff at The Bridges continues
to provide hands on assistance with her activities of daily living, necessary verbal cues,
redirection and 24 hour supervision. According to staff Mrs. Dugan required more hands on

assistance than she did a year ago. Mrs. Dugan also benefits from the activities that the dementia
unit provides on a daily basis. Emotionally she has been fairly stable and has not displayed any
significant behavioral problems.
Mrs. Dugan has done very well over the past year and continues to like her living
arrangement. She attends all the different social activities on a daily basis. Mrs. Dugan enjoys
her surroundings, the staff and the other residents. She is usually very pleasant, has a smile on
her face and enjoys joking around with others. She has made friends and feels very secure in her
living environment. Mary does not like being away for any length of time and is always relieved
when she returns. She does not like being away from home. The staff enjoys having her and she
is cooperative with their requests. Mrs. Dugan has no concerns or complaints regarding her
living situation.
Client is visited every 30-60 days by care management staff. The Bridges provides
scheduling and transportation to all her medical appointments. The staff at The Bridges provides
updates and notification of any changes that Mrs. Dugan might experience. Connie Stoneroad of
Keystone Guardianship Services handles all of her financial affairs. The Office of Aging receives
money from Keystone to purchase personal items for Mrs. Dugan. A few former neighbors
continue to visit regularly and family who lives locally have seen her twice this year. Her
daughter in Florida calls twice a week on Sundays and Wednesdays. Family is informed of any
significant changes in Mrs. Dugan's life.
It is the request of the Cumberland County Office of Aging that the Agency retains Guardianship
of Person for Mary Dugan. The Bridges at Bent Creek has demonstrated an adequate quality of
care for client by managing her daily needs and continuously monitoring her medical conditions.
Mrs. Dugan's appearance is always good and she appears to be very content in her present living
situation. Therefore, we believe that Mrs. Dugan should continue to reside at The Bridges of
Bent Creek.
