1. Plaintiff Law Office of Sam Street, PLLC. is a law firm and collection agency located at
2. P O Box 420670, Houston, Texas.
3. CACH, LLC. is a debt collection firm and financial services firm located at 307 17tH
Street, Suite 5000, Denver, Colorado.
4. Defendant Heather Kauffman Wilt is an adult individual last known to be residing in
Cumberland County Pennsylvania.
5. At some time prior to this date Heather Kauffman Wilt did obtain a Fleet Bank Card,
Account #4326-8100-5501-8973 and did utilize said card to make purchase of
merchandise and/or services with an outstanding balance of $3068.07.
6. At all times relative hereto, the Fleet Bank Account at the above captioned account
number was transferred to Bank of America and thereafter sold to Plaintiff CACH, LLC.
and thereafter assigned for collection to Plaintiff Law Office of Sam Streeter, PLLC.
7. Despite repeated demands Heather Kauffman Wilt has refused to pay all indebtedness,
interest, fees, costs and expenses on this account.
WHEREFORE, for all the above reasons, the Plaintiffs request judgment against
Heather Kauffman Wilt in an amount of $3068.07.
