I, ROBERT G. FREY, Esquire, do hereby swear that service of the Petition was
made on the alleged incapacitated person, Richard L. Piper, by delivery of a copy to him
and an explanation of its contents by me on May 16, 2008. In addition, I read the entire
contents of the Petition to him on June 5, 2008. Copies of the Petition were sent to all of
his next of kin as enumerated at paragraph number three of the Petition.
