Notice is hereby given that The Carter-Jones Lumber Company with its principle place of
business located at 601 Tallmadge Road, in the City of Kent, State of Ohio, and a place of business
at 1840 Harrisburg Pike, Carlisle, Pennsylvania 17013, has and claims a lien against Kevin Walters
and Katrina Hivner, owners of the real property hereinafter described, on said real property for the
sum of Eighteen Thousand, Four Hundred Sixteen Dollars and Sixty-Four Cents ($18,416.64), being
the balance remaining for the value of certain materials furnished for the improvement of said
property.

The materials were furnished with the knowledge and consent, and at the request, of the
owners of the hereinafter described real property.
The interest of the owners in said real property as far as is known to this lienor is fee simple
absolute.
Lienor, a building/plumbing/heating materials supplier, entered into an "H Account" with
o~vners, under which account lienor agreed to furnish materials for the improvement of the aforesaid
property located at Lot #29 Greystone Acres, in West Pennsboro Township, County of Cumberland,
Commonwealth of Pennsylvania.
In the performance of said account, lienor furnished all materials required to be furnished by
it under the account.
Ali of the materials required to be furnished by said lienor have been furnished.
The amount unpaid and owing to this lienor from the owners under the account is Eighteen
Thousand, Four Hundred sixteen Dollars and Sixty-four Cents ($18,416.64). The first item of
materials furnished was on or about August 27, 2003. The last item of materials was furnished on
or about November 21, 2003.

Formal Written Notice of Intention to File Claim was sent via certified mail to the above-
named Defendants on or about January 29, 2004, a copy of which is attached hereto as Exhibit "A"
and incorporated herein by reference as if set forth in full.
The real property subject to such lien and for the improvement of which said services were
performed and said materials furnished is briefly described as follows:

You are hereby notified that The Carter-Jones Lumber Company is a provider of building supplies
and has furnished labor and/or materials in connection with the erection, construction, alteration or repair
or an improvement to your property known as Lot #29, Greystone Acres, West Pennsboro Township,
Cumberland County, Pennsylvania.
The labor and materials furnished by The Carter-Jones Lumber Company consisted of building
supplies and materials. The last invoice is dated November 21,2003. The amount still due and payable
by you to The Carter-Jones Lumber Company is $18,416.64.
Unless payment of the amount due is made within thirty (30) days after service of this formal
notice upon you, The Carter-Jones Lumber Company intends to file a Mechanic's Lien Claim for this
amount against your property, and the proceedings to enforce the lien may result in a judgment against
you and the forced sale of your property.
