PETITION FOR APPOINTMENT OF LEGAL GUARDIAN
AND NOW, the Petitioner, Shannon E. Weaver, through his attorney, Dawn S. Sunday,
Esquire, files this Petition for Appointment of a Legal Guardian over the person of Grady James
Weaver, a minor under the age of fourteen years, and in support thereof, respectfully represents as
follows:
1. The Petitioner is Shannon E. Weaver, an adult individual, residing at 612 South View Drive,
Mechanicsburg, Cumberland County, Pennsylvania 17055.
2. The minor Child over whose person the Guardianship is sought is newborn, Grady James
Weaver. The Child's address is 612 South View Drive, Mechanicsburg,
Cumberland County, Pennsylvania 17055.
3. The Petitioner is the maternal grandfather of the Child.
4. The biological Mother of the Child is Amanda S. Weaver, who resides at 612 South View
Drive, Mechanicsburg, Cumberland County, Pennsylvania 17055.
5. The biological Father of the Child is undetermined at this time.
6. The Child's biological Mother, Amanda S. Weaver, consents to this Petition.
7. The Child's maternal grandmother and the Petitioner's spouse, Barbara E. Weaver, consents
to this Petition.
8. The appointment of the Petitioner as Legal Guardian of the Child is required by the
Petitioner's employer in order for the Child to be covered under the
Petitioner's medical insurance plan. The Child's Mother, who is a student and a dependent
of the Petitioner, is covered under the Petitioner's medical insurance plan.
9. The religious persuasion of both the Child's Mother and the Petitioner is Lutheran.
10. The proposed Legal Guardian's written consent to the appointment is attached hereto as
Exhibit A and the written consent of the Child's Mother to the appointment is attached
hereto as Exhibit B.
WHEREFORE, The Petitioner, Shannon E. Weaver, respectfully requests that this
Honorable Court grant the Petition and enter an Order appointing him as Legal Guardian of Grady
James Weaver.

I agree to accept the appointment as Legal Guardian of the person of Grady James Weaver.
My domicile is 612 Southview Drive, Mechanicsburg, Cumberland County, Pennsylvania.
I am a citizen of the United States.
It is not my intention to apply for an allowance for the support or education of the minor during
minority.
