PETITION FOR CHANGE OF NAME TO THE HONORABLE JUDGES OF SAID COURT: The Petition of Tamila L. Lay on behalf of her minor child, Karl Edward Lay, a/k/a Karl Edward Krehling, by her attorney, Mark T. Silliker, Esquire, respectfully represents the following: 1. Petitioner is Tamila L. Lay, an adult individual who resides at 11 East Glenwood Drive, Camp Hill, Cumberland County, Pennsylvania 17011. The subject minor child, Karl Edward Lay, a/k/a Karl Edward Krehling, also resides at 11 East Glenwood Drive, Camp Hill, Cumberland County, Pennsylvania 17011. The subject minor child was born on March 1, 1992. Likewise, Petitioner Tamila L. Lay is the mother of the subject minor child. 2. The subject minor child, Karl Edward Lay, a/k/a Karl Edward Krehling, has resided at the following addresses for the five (5) years previous to the filing of this Petition: a. 11 East Glenwood Drive, Camp Hill, Cumberland County, Pennsylvania, 17011. 3. Petitioner desires and intends to change the name of Karl Edward Krehling to Karl Edward Lay. 4. Petitioner provides the following reasons for this request: The last name of the subject minor child has already been changed by agreement of the child's natural parents. Indeed, a new Birth Certificate has been issued by the Pennsylvania Department of Vital Records. However, the federal government will not issue a new social security card in the child's last name of Lay unless there is an actual court proceeding followed by a Court Order officially changing the child's last name. It is respectfully submitted that in terms of state law, the child's last name has already been changed, consistent with state law as well as regulations set forth by the Department of Vital Records. However, Petitioner would also request the approval of this Court in order to satisfy the requirements of the Social Security Administration. 5. The proposed change in the subject minor child's name if granted will not be detrimental to the interests of any other person and it not against public interest. 6. The Petition is not filed to defraud creditors. 7. The subject minor child does not own any property, either real or personal. 8. A photocopy of the child's previous Birth Certificate which was issued at birth under the name of Krehling is attached hereto and marked Exhibit "A". As well, a photocopy of the back of the Birth Certificate is also attached, clearly setting forth the consent of both natural parents, Daniel L. Krehling and Tamila L. Lay, to this change of name. Likewise, the present, modified, and otherwise valid Birth Certificate of the subject minor child issued November 19, 2007 pursuant to the agreement of the natural parents is also attached hereto and marked Exhibit 461399. WHEREFORE, Petitioner requests this Honorable Court pursuant to the Act of December 16, 1982, P.L. 1309, No. 295, § 2, 54 Pa.C.S. § 701, for the following: a. to fix a hearing on this Petition; b. to direct that notice be given on this Petition and hearing; and c. to enter a decree changing the subject minor child's name from Karl Edward Krehling to Karl Edward Lay.
