AND NOW, this 17th day of June 2008 comes the guardian,
Constance E. Stoneroad, of Keystone Guardianship Services, of
the above-captioned incapacitated individual, petitions as
follows:
1. Keystone Guardianship Services was appointed Guardian
for Janette Neely, an alleged incapacitated person by the
Cumberland Country Orphans' Court by order dated March 5th 2008.
(see copy of Order attached as Exhibit "A").
2. Janette Neely is presently a resident of Arden Courts
of Susquehanna, located at 2625 Ailanthus Lane, Harrisburg,
Dauphin County Pennsylvania 17110.
3. Janette Neely has been taken from Arden Courts by her
Guardian, Keystone Guardianship Services for doctor
appointments, surgery, lunches and other all day outings with
the Guardian for pool parties, picnic, etc.
4. Janette Neely has also been taken from Arden Courts by
Betty Schlusser, for lunch and shopping.
5. Keystone Guardianship Services representatives and
Betty Schlusser have had no adverse experiences while out with
Janette Neely. She has been agreeable, friendly and quite
pleasant to be with during the occasions she has been away from
Arden Courts.

6. Gladys and Henry Wills, of 345 Sandy View Lane,
Harrisburg, Dauphin County, Pennsylvania, longtime friends of
Janette Neely, have visited with Janette on a regular basis from
the time since Ms. Neely has became a resident at Arden Court.
7. Gladys and Henry Wi11s have taken in and are presently
housing Janette Nee1y's horses, which were not being cared for
by Janette's daughter, Sandra Nestor.
8. Gladys and Henry Wi11s live a short distance from
Janette Neely and can readily take her to their home for visits.
9. The Guardian has advised Gladys and Henry Wi11s of the
need to have Ms Neely's emergency medical information with them
at all times and have also been advised that if any kind of
medical emergency or incident takes place, Keystone Guardianship
Services must be contacted immediately.
10. The Guardian has advised Gladys and Henry Wi11s of the
individuals that the Guardian feels are not to be present during
Ms Nee1y's visits with them.
11. At the time of Keystone Guardianship Services'
appointment, it was necessary to allow only Keystone
Guardianship Services' to take Ms. Neely from Arden Courts. As
Ms. Neely has taken prescribed medications on a regular schedule
for her physical and mental wellness, her Guardian has noticed a
marked improvement in her attitude and behavior.

WHEREFORE, the aforementioned guardian respectfully requests
this Honorable Court enter an Order directing the following:
To allow Henry Wills and Gladys Wills to take Janette Neely
from Arden Court for visits at their home and other
miscellaneous activities.

. (1} Janette Neely Is adjudicated an incapacitated person in need of a plenary
guardian of Fier person and plenary guardian of her estate.
(2). Keystone Guardianship Services NPC is appointed plenary guardian of the
person of Janette Neely and plenary guardian of the estate of Janette Neely,
(3} Keyston® Guardianship Services NPC shad be paid for its services in
. accordance with its regular fee schedule.
(4) As guardian of the estate of Janette Ne®ly, (1) Keystone Guardianship
Services NPC, may, without prior court approval, invade principal as necessary for
payment of expenses and debts and (2) is authorized to ent®r into listing Contracts for
the sale of real estate; however, court approval shalt be required prior to signing of any
contract of sale.
(5) No party oth®r than the guardian of the person shall have the right to remove

Bayi~y, J. March 5, 2008:- .
Janette Neely, age i34, is currently a resident at Arden Court, ~ Harrisburg,
Pennsylvania, a facility specializing in the care of Alzheimer patients. We find that due
- to har Alzheimer dlseas®, her abitrty to receive and evaluate information effectively and
communicate decision are impaired to such a significant extent that she is unable to
.manage her financial resources and unable to meet essential requirements for her
physical heath and safety. Her prognosis is for further~mental' detsrloration. We fiind
that she is an incapacitated person in need of a plenary guardian of her persari~ and
plenary guardian of her estate for an indefinite period, ~Ne®ly, who is represented by
private counsel, requests that Ke~rstone Guardianship Services NPC be appointed
pl®nary guardian of her p®rson and plenary guardian of her estate. Based on the above
findings, the following order is entered.

person of Janette Neely and plenary guardian of f3~e estate of Janette Neely.
(3) Keyston® Guardianship Services NPC shalt be paid for its services in
accordanc:® with its regular fee schedule,
(4) As,guardia~n of the estate of Janette Neely, {~) Keystone Guardianship

representatives of Arden Court sfiall have the authority to use their discretion which may
include restriction of visits or other contact with the incapacitated p®rsan.

requirements set forth in 20 Pa.C.S.A. Section 629 (c).
