AND TO APPOINT A GUARDIAN FOR HER PERSON AND HER ESTATE
PRELIMINARY DECREE

to have ABBE S. GRIPPIN adjudicated an incapacitated person and to have a GUARDIAN
appointed for her person and her estate. Notice of the hearing shall be give to ABBE S.
GRIPPIN in accordance with 20 P.S. 5511 (a"j not less than twenty (20) days prior to the hearing.
