AND NOW comes Charles S. Blichasz, resident of 652 Deer Road, Boiling Springs,
Cumberland County, Pennsylvania, and requests this Honorable Court for an order and avers as
follows:
1. On July 18, 2007 the undersigned purchased a 2003 Hitong Scooter, VIN #
LBFTCBDK722002789 as per Surplus Auctions, LLC acting as agent of the State of
Pennsylvania for the disbursement of surplus and/or seized property.
2. Pursuant to this purchase, petitioner now requests that this Honorable Court issue a
court order requesting the Pennsylvania Department of Transportation to grant a certificate of
title.
3. Information about the vehicle is as follows:
A: Year: 2003
B: Model: HIT 49SR3
C: VIN #: LBFTCBKD722002789
D: Engine #: LJ139QMB020216828
4: Enclosed is a Bill of Sale as Exhibit A for the aforementioned vehicle.
5: Enclosed is a copy of the Hitong Scooter Qualification Card #020780 as Exhibit B.
6: Enclosed is a pencil rubbing of the VIN # from the Scooter frame as Exhibit C.
WHEREFORE, Petitioner requests the Honorable Court to grant the attached order.

IN RE: 2003 HITONG SCOOTER
ORDER OF COURT
AND NOW, this 9th day of July, 2008, the Court hereby awards ownership of a
2003 Hitong Scooter, Model 1-11T 39SR3, bearing VIN#:LBFTCBDK722002789, to
a
Petitioner, Charles S. Blichasz, and the right, title and interest of any other person to
said vehicle is hereby extinguished. The Commonwealth of Pennsylvania, Department
of Transportation may accept this Order of Court as evidence of ownership in lieu of
Certificate of Title. The Petitioner shall submit the appropriate forms, taxes and fees and
comply with any other procedure of the Commonwealth of Pennsylvania, Department of
Transportation, in order to receive the appropriate Certificate of Title for said vehicle.
