~, Petitioner JOAN M. BEATTIE is an adult individual who resides at ~ Partridge
Circle, Carlisle, Cumberland County, Pennsylvania 17013.

THEODORE RUDBERG is an adult individual who was born on June 1, 1938,
and has been hospitalized at Holy Spirit Hospital, 503 North 21st Street, Camp Hill,
Cumberland County, Pennsylvania 17011, for more than ten days due to problems with a
dialysis port, secondary to renal disease.

Although Theodore Rudberg is physically and mentally capable of completing a
marriage application, it is not presently possible to determine how long it will be before
his present medical problems will be resolved and he will be able to be released from the
hospital.

Joan M. Beattie and Theodore Rudberg have resided together for more than ten
years, and they desire to marry.
WHEREFORE, Petitioner respectfully requests that your Honorable Court issue an
Order authorizing and directing the deputy clerk of the Orphans' Court Division of the
Court of Common Pleas of Cumberland County, Pennsylvania, to take the marriage
application of Joan M. Beattie and Theodore Rudberg at Holy Spirit Hospital.
