FINAL DECREE
AND NOW, this 19th day of June, 2008, upon
consideration of the annexed petition and after hearing held
following due notice, it is hereby ORDERED and DECREED that, based
upon a finding that Richard L. Piper suffers from cerebral palsy
and as a result is so severely mentally impaired that he is unable
to make, communicate, or even participate in any decision relating
to his estate or person, Richard L. Piper is adjudged totally
incapacitated. Edgar L. Piper and Nancy L. Piper are appointed
plenary guardians of the person and plenary guardians of the estate
of Richard L. Piper.
The respective guardians shall file with this Court a
report within 12 months from the date of this decree and at least
annually thereafter which

Richard L. Piper, the incapacitated person;
(ii) major medical ar mental problems of Richard
L. Piper, the incapacitated person;
(iii) a brief description of the living
arrangements of Richard L. Piper, the incapacitated person, and the
:social, medical, psychological, and other support services he is
receiving;
(iv) the opinion of Edgar L. Piper and Nancy L.
F?iper as to whether the guardianship should continue or be
germinated or modified, and the reasons therefore; and
(iv) the number and length of times Edgar L. Piper
and Nancy L. Piper visited the incapacitated person in the past
year.
No bond shall be required of the co-guardians.
