Cook Revocable Trust Agreement, originally dated January 18, 1972, as subsequently amended and restated in its entirety on November 16, 2004 (the "Trust"), you are hereby notified of the following particulars: 1. The Trust was created by Louise V. Cook, as Settlor, by that certain Revocable Trust Agreement originally dated January 18, 1972, with Dauphin Deposit Bank & Trust Company (now, by merger, Manufacturers and Traders Trust Company) as Trustee. Article VI(1) ofthe Restated Trust Agreement provides, as follows: Upon the death of the Settlor, Susan L. Cook shall become an additional Trustee (the "Individual Trustee") by delivering to the Corporate Trustee her written acceptance of the office. 2. Susan L. Cook has executed a written acceptance of her appointment as Co- Trustee. A copy of the Restated Trust Agreement, together with a copy of Susan L. Cook's Acceptance of Appointment as Co-Trustee, is furnished to you herewith. 3. Louise V. Cook, the Settlor, died on May 31, 2008, a resident of Upper Allen Township, Cumberland County, Pennsylvania. The Register of Wills of Cumberland County, Pennsylvania, has issued Letters Testamentary to M&T Bank and Susan L. Cook, as Executors of the Last Will of Louise V. Cook, deceased. 4.
