with Notice of Hearing, upon JUANA CONCEPCION, reading and explaining the same
to her and leaving a copy thereof with JUANA CONCEPCION on the day of
June, 2008, at 1 a:(J6" p.M., at her place of residence at Golden Living Center, 770
Poplar Church Road, Camp Hill, Pennsylvania.
I understand that the statements made herein are subject to the penalties set
forth in 18 Pa. C.S.A. §4904 relating to unsworn falsification to authorities.
