Pelella, by and through her attorney, Marylou Matas, Esquire, and petitions this Honorable Court as follows: 1. Petitioner is Giovanna C. Pelella, an adult individual who resides at 7085 Carlisle Pike, Carlisle, Cumberland County, Pennsylvania 17015. 2. Petitioner was born on February 24, 1935 in Caserta, Italy. A copy of her birth certificate is attached hereto and incorporated herein by reference as Exhibit A. 3. Petitioner immigrated to the United States at age 15, taking the first name of "Jean" upon her arrival, at the urging of relatives. 4. Petitioner is known to the general public, friends and community as "Jean." 5. Legal documents are issued in alternate names of "Jean" and "Giovanna," however. 4. Petitioner was married to her husband, Ronald Weiss, on October 16, 1960, using the name Jean C. Pelella. A copy of their marriage certificate is attached hereto and incorporated herein by reference as Exhibit B. 5. Petitioner's Pennsylvania identification card is issued in the name of Giovanna SAIDIS, FLOWER & LENDS" 26 West High Street Carlisle, PA C. Weiss. A copy of said ID card is attached hereto and incorporated herein by reference as Exhibit C. 6. Petitioner's social security card is issued in the name of Jean C. Weiss. A copy of said card is attached hereto and incorporated herein by reference as Exhibit D. 7. Petitioner's passport is issued in the name of Giovanna C. Weiss. A copy of said card is attached hereto and incorporated herein by reference as Exhibit E. 8. Petitioner desires for this matter to be made clear and for her legal name to be made "Jean C. Weis" for all intents and purposes. WHEREFORE, Petitioner prays this Honorable Court to hold a hearing and, after the hearing, to order that the name of Giovanna C. Pelella be changed to Jean C.
