Decedent, Daniel N. Thundu was domiciled at death in Cumberland County, Pennsylvania, with his last family or principal~residence located at 1602 West Lisburn Road, Mechanicsburg, Upper Allen Township, Pennsylvania. 2. The Decedent was involved in an automobile ·accident occurring on January 14, 2000, wherein he suffered multiple traumatic injuries causing his death at approximately 11:10 p.m. on said date. 3. A certified copy of the Decedent's Death Certificate is attached hereto, incorporated herein by reference and marked as Exhibit "A". 5. It is believed and therefore averred that the Decedent, at the time of his death, owned no real property situated in the Commonwealth of Pennsylvania. 6. It is believed and therefore averred that the Decedent, at t~e time of his death, owned personal property in Cumberland County in the approximate amount of $500.00. 7. No Letters of Administration have been taken out nor has an Estate been opened with regard to the Decedent in ~Cumberland County or any other county within the Commonwealth. 8. Decedent was survived by his wife, Susan (Sarvis) Thundu and a minor daughter, both whom reside at 31 Ashburg Drive, Mechanicsburg, Silver Spring Township, Cumberland County, Pennsylvania 17055. 9. Petitioner is not aware of any other heirs of Decedent's Estate. 10. It is necessary for an estate to be opened for the purposes of litigation. 11.
