PETITION FOR GUARDIANSHIP AND NOW, comes Petitioner, Mary Zayas-Bazan,' by and through her attorney, Heather L. Harbaugh, Esquire, of Edward J. Weintraub & Associates, and hereby files this Petition for Guardianship, and in support thereof avers the following: 1. The subject of this petition is the minor child, Rene Ryan Zayas-Bazan, age 14, date of birth January 28, 1987, who currently resides at 154 Faith Circle, Carlisle, Cumberland County, Pennsylvania. Rene Ryan Zayas-Bazan has resided in Pennsylvania for the past 13 years. The minor does not have any appointed guardian at this time. 2. The natural mother of Rene Ryan Zayas-Bazan is Mary Zayas-Bazan (hereinafter "Mother"), age 37, date of birth January 11, 1964, who resides at 154 Faith Circle, Carlisle. She has resided in Pennsylvania for the past 13 years. The minor child, Rene Ryan Zayas-Bazan has lived with Mother since birth. 3. The natural father of Rene Ryan Zayas-Bazan is Rene Zayas-Bazan, age 43, date of birth December 28, 1958, whose address is 8371 South West 28th Street, Miami, Florida. with the child. Father has been paying child support for the child and occasionally visits 4. The proposed guardian is Sheila Brown, Mother's sister and the child's aunt, age 44, date of birth March 29, 1957, who resides at 12020 Willow Grove Road, Camden, Delaware. 5. guardian of the minor child. His written consent is attached hereto as Exhibit "A." 6. Mother'and the proposed guardian, Sheila Brown, agree that it is in the child's best interest that he reside with his aunt, Sheila Brown, in Delaware and that he be enrolled in school in Delaware. The executed Agreement for Appointment of Guardian is attached hereto as Exhibit "B. 7. The proposed guardian, Sheila Brown, consents to being the child's guardian. AGREEMENT FOR APPOINTMENT OF GUARDIAN WHEREAS, the proposed guardian, Sheila Brown, is the biological sister of the Petitioner, Mary Zayas-Bazan~ and the biological aunt of the minor Child, Rene RYan Zayas- Bazan, date of birth, January 28, 1987; and Whereas, Petitioner, the child's mother, intends that the child shall live with the proposed guardian, Sheila Brown, so that the child may attend school in Delaware; and Whereas the proposed guardian, Sheila Brown, lives at 12020 Willow Grove Road, Camden, Delaware; and' Whereas the.child, Rene Ryan Zayas-Bazan, has resided with his mother in Carlisle, PA for most of his natural life; and Whereas, Rene Zayas-Bazan, the natural father of the minor child, consents to the appointment of Sheila Brown as guardian of the child; and Whereas, Petitioner and the proposed guardian desire to make arrangements for the guardianship of the minor child so that the child may attend school in Delaware; and Whereas all parties hereto have been advised by counsel or have had the opportunity to be so advised by counsel; and Whereas, the parties intend to submit this Agreement to the court of appropriate jurisdiction for merger into a court order approving said Agreement; NOW, THEREFORE, it is hereby stipulated and agreed as follows: 1.
