AND NOW, this ~'~ day of 2008, in consideration of the
parties' Stipulated Agreement, it is her y ORDERED AND DECREED that:
1. Margaret Bauerle suffers from advanced dementia and is
unable to receive and evaluate information effectively and
make and communicate decisions regarding the
management of her financial affairs.
2. The Guardian of the Estate of Margaret Bauerle for Medical
Assistance purposes only shall be Neighborhood Services,
located at P.O. Box 1593, Lancaster, Pennsylvania, 17608.
3. The Guardian of the Estate of Margaret Bauerle for Medical
Assistance purposes shall not dispose of any assets being
held by Margaret Bauerle in the form of stocks, bonds, CDs,
IRAs, or annuities without further Order of Court.
4. The Guardian of the Estate of Margaret Bauerle for Medical
Assistance purposes shall not communicate any statements
regarding ownership of stocks, bonds, CDs, IRAs, or
annuities that may or may not be held by Margaret Bauerle.
5. J. Marc Bauerle shall be the Guardian of the Person of
Margaret Bauerle and Guardian of the remainder of
Margaret Bauerle's estate.
6. J. Marc Bauerle shall not dispose of any assets being held by
Margaret Bauerle without further Order of Court.
The Orphans' Court is hereby directed to enter this Order of Record, and to mail a copy
of this Order to all counsel of Record p ant to PA .C.P. 236.
