PENNSYLVANIA PETITION FOR CHANGE OF NAME TO THE HONORABLE, THE JUDGES OF SAID COURT: The Petition of Richard William Betz, by his attorneys, Keefer Wood Allen & Rahal, LLP, respectfully represents: 1. Richard William Betz was born on February 14, 1979, in Dauphin County, Pennsylvania, to Sheila Farris and Richard William Maxwell, Sr. 2. The Petitioner desires to change his name so that his birth certificate, social security card, and other identifying documents bear the name of Richard Moxie Maxwell. 3. The Petitioner resides at 219 South Enola Drive, Enola, Cumberland County, Pennsylvania, and has resided there since October 2002. 4. Prior to establishing his current address, the Petitioner resided at 4020 Crooked Hill Road, Harrisburg, Dauphin County, Pennsylvania, from July 2002 to October 2002. 5. Prior to establishing the address listed in Paragraph 4 above, the Petitioner has resided at 130 Beaver Road, Apartment B, Harrisburg, Lower Paxton Township, Dauphin County, Pennsylvania, from June 2001 to July 2002. 6. Prior to establishing the address listed in Paragraph 5 above, the Petitioner resided at 4020 Crooked Hill Road, Harrisburg, Dauphin County, Pennsylvania from March 1999 to June 2001. 7. Prior to establishing the address listed in Paragraph 6 above, the Petitioner resided at 1925 North 7th Street, Harrisburg, Dauphin County, Pennsylvania for a period of time which began at least five (5) years preceding the filing of this Petition, and which ended in March 1999. 8. Petitioner learned from his natural mother that his natural father's name was Richard William Maxwell, Sr. and that his natural parents were never married to each other. 9. Because Petitioner desires his name to reflect his true identity and heritage and because of his desire to have any children or other descendants bear his family name, Petitioner desires the above-referenced name change. 10. There are no judgments or decrees of like character of record against the Petitioner. Copies of the results ofjudgrnent searches in Dauphin and Cumberland Counties regarding Petitioner are attached as Exhibit "A" and incorporated herein. WHEREFORE, the Petitioner requests the Court, pursuant to the Act of December 16, 1982, P.L. 1309, No. 295, § 2, 54 Pa. Cons. Stat. § 702, to: a. Fix a heating on this Petition; b. Direct that notice be given of this Petition and heating; and Enter a decree changing the name of the Petitioner from Richard William Betz to Richard Moxie Maxwell.
