CITATION WITH NOTICE
A petition has been filed with the Court to have you declared an Incapacitated Person If the
Court finds you to be an Incapacitated Person, your rights will be affected, including your right to
manage money and property and to make decisions A copy of the petition which has been filed by
Terry L Heffelfinger is attached
You are hereby ordered to appear at a hearing to be held ~n Court Room No 3, Cumberland
County Courthouse, Carlisle, Pennsylvania, on March 29 ,2004, at 9 30 A M to tell the
Court why is should not find you to be an ~ncapacltated Person and appoint a Guardian to act on your
behalf
To be an incapacitated Person means that you are not able to receive and
effectively evaluate information and communicate decisions and that you are unable to
manage your money and/or other property, or to make necessary decisions about where
you will live, what medmal care you will get, or how your money will be spent
At the heanng, you have the right to appear, to be represented by an attorney, and
to request ajury trial If you do not have an attorney, you have the right to request the
Court to appoint an attorney to represent you and to have the attorney's fees prod for you
if you cannot afford to pay them yourself You also have the right to request that the
Court order that an independent evaluation as to your alleged incapacity
If the Court decides that you are an Incapacitated person, the Court may appmnt a
Guardian for you, based on the nature of any condition or disability and your capacity to
make and communicate decisions The Guardian will be of your person and/or your
money and other property and will have either limited of full powers to act for you
If the court finds you are totally incapacitated, your legal rights will be affected
and you wall not be able to make a contract or gift of your money to other property If the
court finds that you are partially incapacitated, your legal rights will also be hm~ted as
directed by the Court
If you do not appear at the hearing (either in person or by an attorney representing you)
the court will stall hold the heanng in your absence and may appoint the Guardian requested'

alleged incapacitated person ~s currently an ~npat~ent at the Holy
Spent Hospital, 503 N 21st Street, Camp Hdl, PA 17011 and ~s
deemed by her physm~ans to be unable to safely travel

~ncapacitated person has three chddren Terry L Heffelflnger,

The alleged incapacitated person ~s currently being treated by
Dr Mana Herrada and Dr David Petkash both of Holy Spent
Hospital, 503 N 21st Street, Camp Hill, PA and Dr David
Peters, 899 Poplar Church Road, Camp H~II, PA 17011
The Pebboner ~s Terry L Heffelflnger, residing at 119 C~rcle
Dnve, Hummelstown, Pennsylvama who ~s the daughter of the
alleged ~ncapac~tated person The Pebboner has no ~nterest
adverse to the alleged ~ncapac~tated person and seeks to be
appointed Guardian Ad L~tem to ensure the alleged
~ncapac~tated person's conbnued physical and mental health
treatment.
The alleged ~ncapac~tated person has been d~agnosed by Dr
Mana Herrada w~th Psychotic D~sorder and by Dr Dawd Peters
w~th Esophageal Stncture
The Pebt~oner alleges that the alleged incapac~tated person has
shown herself to be unable to adequately care for her own
needs and would hkely not pursue any physical or mental health
treatment as prescribed by Drs Harrada, Petkash and Peters
The Pebboner requests that she be appointed Guardian Ad
L~tem of the alleged ~ncapac~tated person to assure continued
needed physical and mental health treatment and over the
estate of the alleged incapacitated person to ensure that the

The Pebt~oner alleges that she ~s the most quahfied ~nd~v~dual to
act as the Guardian Ad L~tem of the alleged ~ncapac~tated
person hawng her best ~nterests ~n mind
The Pet~boner seeks to be appointed the Guardian Ad L~tem of
the alleged ~ncapac~tated person only so long as the alleged
~ncapac~tated person ~s determined by her treating doctors, Drs
Herrada, Petkash and Peters to be unable to take care of her
own needs
The Pebboner esbmates that the gross value of the alleged
~ncapacltated person estate ~s $10,000 00 and her current
~ncome $900 00 per month from Social Security
