Gan, Esquire and makes the following averments in support of this Petition for Name Change pursuant to 54 Pa C.S. 702 pertaining to judicial change of name: 1. Alexander D. Daraban is an adult with a date of birth of 1/14/1938. Please see Birth Certificate attached hereto, marked Exhibit A and incorporated by reference. 2. The Petitioner resides at 314 N. 25m Street, Camp Hill, Pa 17011. 3. The Petitioners social security number is 194 68 3380. Please see a copy of Petitioners social security card which is attached hereto and marked exhibit B. 4. The Petitioner desires to restore his birth name which is Alexandru D. Daraban. 6. The Petition for Name change is not for fraudulent purposes and in the best interest of the Petitioner. 7. There is no social stigma and the use of this name in the community is neutral and therefore not an issue. 8. Petitioner has been fingerprinted in accordance with the judicial change of name requirements and his fingerprint card is attached hereto as Exhibit C.
