1. Petitioner Amber Dee Tittle desires to change her name and honor a
teacher who had a significant impact upon her life.
2. Petitioner is an adult resident of Cumberland County, Pennsylvania residing
at 427 Oak Flat Road, Newville, Pennsylvania.
During the past five years, Petitioner has resided at the following
residences:

4. Petitioner desires to change her name to Amber Dee Vallejo.
WHEREFORE, Petitioner requests this Court to schedule a hearing and thereafter
grant her request to change her name from Amber Dee Tittle to Amber Dee Vallejo.

fingerprints to the Pennsylvania State Police, submission of an official search of the proper
offices of this county showing that there are no judgment or decrees of record or any
other matter of like character against said Petitioner and proper notice of this hearing
being provided, the Court is satisfied that Petitioner has fulfilled all of the requirements for
a judicial change of name. It is therefore ordered that from this time forward Amber Dee
Tittle shall be known as Amber Dee Vallejo.
