attached Petition for Modification and Amendment of Birth Certificate, the Court finds that
sufficient grounds exist to establish a modification of the paternity of the minor child, and that
Petitioner THAI D. BARRICK shall be replaced as the named father on the Birth Certificate.
NOW, THEREFORE, it is Ordered and Decreed that the Chief, Division of Vital
Statistics, and State Registrar of the Department of Health of the Commonwealth of
Pennsylvania shall amend birth registration No. 0421890 - 2004 so that the name of the child
shall be changed from Hailey Brynn Ewing to HAILEY BRYNN BARRICK, to reflect the name
of the father of the minor child, and so that THAI D. BARRICK be added to the registration as
the father along with the following information: date of birth, February 5, 1984; age, 24; and
race, Caucasian.
