Sangun Nam is a minor, currently 13 years of age and is a bona fide resident of the County of Cumberland, Commonwealth of Pennsylvania, whose residence address is 1013 Saffron Drive, Mechanicsburg, Pennsylvania, 17050, and has permanent resident status in the United States. 2. Meekyoung Oh, an adult individual and mother of Sangun Nam, is the parent and guardian of Sangun Nam, and is a bona fide resident of the County of Cumberland, Commonwealth of Pennsylvania, whose residence is 1013 Saffron Drive, Mechanicsburg, Pennsylvania 17050 and has permanent resident status in the United States. 3. Yong Kwon, an adult individual married to Meekyoung Oh and step father of Sargun Nam is a bona fide resident of the County of Cumberland, Commonwealth of Pennsylvania, whose residence is 1013 Saffron Drive, Mechanicsburg, Pennsylvania, 17050, and is a citizen of the United States. 4. Sangun Nam has been a bona fide resident of Cumberland County, State of Pennsylvania for at least one (1) year immediately prior to filing this Petition. 5. Sangun Nam was born on the 27th day of January, 1996, in Seoul, South Korea. 2 6. Petitioner's present name is Sangun Nam. This is the current name of Petitioner on her social security card and all other records. 7. Sangun Nam is a minor and is not married. 8. Sangun Nam has no children. 9. Sangun Nam owns no real estate. 10. Sangun Nam has lived at the following address(es) over the last five (5) years: a. 1013 Saffron Drive, Mechanicsburg, PA 17050, February 2004-Present; b. 10203 Quiet Pond Terrace, Fairfax, VA 22013, January 2003-January 2004. 11. The name change petitioned for by Sangun Nam is supported by her mother and guardian, who has custody of Sangum Nam, her stepfather Yong Kwon with whom she also resides, and her father, Kyoungmin Nam, who is a resident of Seoul, South Korea. Attached hereto as Exhibit "A" is a signed statement by Sangun Nam's father, Kyoungmin Nam, attesting that he agrees to and has no objection to such name change. Mr Nam's statement is in Korean, and a translation is also attached as part of Exhibit "A." 12. Sangun Nam has never been convicted of a felony or a misdemeanor. 13. Sangun Nam has never had any judgments or decrees entered against her, and has no outstanding bills, financial obligations or debts. 14. Sangun Nam prays for the change of name in order to have the same family name as her parents and family. 15. The proposed change in Sangun Nam's name, if granted, will not be detrimental to the interests of any other person and is not against the public interest. 16. This Petition is not filed to defraud creditors. 3 WHEREFORE, Sangun Nam prays that by an Order of Court, Sangun Nam's name be changed to and decreed to be JENNY SANGUN KWON.
