PETITION FOR CI~NGE OF NAME
The Petitioner, Carolyn Ann Martin, through her attorneys,
Cleckner and Fearen, respectfully requests that Your Honorable
Court order a change of name for her pursuant to 54 Pa. C.S.§701 et
seq and in support thereof avers the following:
1. Petitioner is Carolyn Ann Martin, an adult individual who
resides at 475 Country Club Road, Borough of Camp Hill, Cumberland
County, Pennsylvania 17011.
2. Martin is Petitioner's married name. Petitioner was
divorced by decree of Your Honorable Court on November 14, 2002
(Docket No. 02-3183). The maiden name of Petitioner was Carolyn
Ann Phelleps.
3. Petitioner desires to change her name to Carolyn Ann
Piacquadio.
4. Petitioner wishes to be known by the surname Piacquadio
as this was her father's original family surname, which surname was
changed at the time of her paternal grandfather's emigration to the
United States from Italy.
5. Petitioner has no children who would be affected by her
name change.
6. There are no judgments or decrees of like character of
record against Petitioner.
7. Concurrent with the filing of this Petition, the
Petitioner has provided the Office of the Prothonotary with a true
and correct set of fingerprints to be forwarded to the Pennsylvania
State Police pursuant to 54 Pa.C.S.§702(b).
WHEREFORE, Petitioner respectfully asks that Your Honorable
Court enter an order changin~ her name from Carolyn Ann Martin to
Carolyn Ann Piacquadio.

to Carolyn Ann Piacquadio.
