TO THE DEFENDANT: The above named plaintiff(s) asks judgment against you for $3,335.79 together with costs upon the
following claim (Civil fines must include citation of the statute or ordinance violated):
Defendant borrowed money to purchase a vehicle and entered into a written agreement for repayment.
Defendant defaulted under the agreement and as of March 12, 2003, Defendant owes Plaintiff a principal balance of
$2,937.78 less a payment and credits of $853.04. The contract provides that Defendant will pay Plaintiff's reasonable
attorney's fees which are estimated to be $900.00, and interest at the contract rate of 21% or $351.05, for a balance of
$3,335.79.
