Keystone Guardianship Services was appointed Guardian for Janette Neely, an alleged incapacitated person by the Cumberland Country Orphan's Court by order dated March Stn 2008(see copy of Order attached as Exhibit "1"). 2. Janette Neely is presently a resident of Country Meadows, located at 451 Sandhill Road, Hershey, Dauphin County Pennsylvania 17033. 3. Keystone Guardianship Services is in need of funds to pay for continuing care of Janette Neely at Country Meadows, medical expenses and expenses relative to her real estate such as taxes, utilities, insurance and upkeep. Janette Neely is the sole owner of 21 Lost Lane, Jackson Township, Blain, Perry County, Pennsylvania (Parcel No. 090,157.00-34.000) (see copy of Deed attached as Exhibit "2"). 5. The said property has been appraised at value of Ninety Eight Thousand ($98,000.00) Dollars by Gayle L. Arbogast, CRA, Arbogast Appraisal Services (see copy of Appraisal attached as Exhibi t "3 ") . 6. Keystone Guardianship Services has received a statement/offer from Bradley C. Leisure, P O Box 100 Blain, PA 17006, in the amount of One Hundred Thousand ($100,000.00) Dollars "AS IS", not to include personal contents (see copy of the offer attached as Exhibit "4"). 7. Keystone Guardianship Services could not locate a signed agreement referred to above nor could Bradley C. Leisure produce a signed agreement for the sale of the real estate. 8. The bid below Mr. Leisure's statement/offer which Keystone Guardianship Services receive was Forty Five Thousand ($45, 000.00) Dollars (see copy of the offer attached as Exhibit ~~5..) . March 5, 2008:-- Janette Neely, age 84, is currently a resident at Arden Court, Harrisburg, Pennsylvania, a facility specializing in the care of Alzheimer patients. We find that due to her Alzheimer disease, her ability to receive and evaluate information effectively and communicate decision are impaired to such a significant extent that she is unable to manage her financial resources and unable to meet essential requirements for her physical heath and safety.' Her prognosis is for further mental deterioration. We find that she is an incapacitated person in need of a plenary guardian of her person and plenary guardian of her estate for an indefinite period. Neely, who is represented by private counsel, requests that Keystone Guardianship Services NPC be appointed plenary guardian of her person and plenary guardian of her estate. Based on the above findings, the following order is entered. ' She suffers from hypertension, asthma and renal disease.
