PETITION OF BRENDA ANN HURLEY
PETITION FOR CHANGE OF NAME
The Petitioner, Brenda Ann Hurley, through her counsel, Post & Schell, P.C., files this
Petition for Change of Name, and in support thereof, avers the following:
1. Petitioner is Brenda Ann Hurley, an adult individual, currently residing at 208
East Winding Hill Road, Mechanicsburg, Cumberland County, Pennsylvania 17055.
2. Petitioner desires to change her name from "Brenda Ann Hurley" to "Patricia
Brenda Hurley."
3. Petitioner was given the name of "Brenda Ann" at birth.
4. Petitioner has been called "Patricia" for the majority of her lifetime, however, has
used the name "Brenda" for her professional life and for professional certifications, thus creating
confusion.
5. Petitioner's name on Resident Alien Card No. A40220483 appears as "Brenda
Ann Hurley."
6. Petitioner's United Kingdom Passport was issued in the name of "Brenda Ann
Hurley".
7. Petitioner would like to formally change her name to "Patricia Brenda Hurley" to
clear up confusion which has existed pertaining to her identity and legal name.
8. During the past five years, Petitioner's only other residence was 3547 Evergreen
Road, Pittsburgh, Allegheny County, Pennsylvania 15237.
9. The change of name is not made for the purpose of defrauding creditors or others.
WHEREFORE, Petitioner respectfully requests that this Honorable Court change her
name from "Brenda Ann Hurley" to "Patricia Brenda Hurley."
