JUDGMENT NOTE
$84,325.00

FOR VALUE RECEIVED, we promise to pay to the order of CREDENTIAL
LEASING CORPORATION the sum of Eighty-Four Thousand Three Hundred Twenty-Five
($84,325,00) Dollars, without offset, together with interest at the rate of eleven and one-half
(11 1/2%) percent per annum, payable in forty-eight (48) equallllonthly installments ofT: ~

and the remaining insta Iments on the ~t day of each month thereafter. All payments shall be
made by wire/electronic funds transfer. We will have fifteen (15) days from each due date to
make payment. Ifwe do not make payment within said fifteen (15) day period, we will receive a
notice of default from Credential Leasing Corporation. Ifwe do not make payment within fifteen
(15) days from the receipt of said notice of default, we will then be in default.
In the event of default, all remaining installments shall, at the option of Credential
Leasing Corporation, and without notice, become immediately due and payable, In the event of
default, this Note may be placed in the hands of an attorney for collection, If this Note is placed
in the hands of an attorney for collection, we agree to pay as reasonable attorney's fees 15% of
the amount due and owing on this defaulted Note,
To secure payment of this Note, we hereby authorize, irrevocably, the Prothonotary,
Clerk of Court or any attorney of any court of record to appear for us in such court at any time
after default and confess judgment against us in favor of any holder ofthis Note with or without
the filing of an Averment of Default, with release of errors, without stay of execution, and for
such amount as appears above, together with charges, attorney's fees and costs as herein

provided, and we hereby waive and release all benefit and relief from any and all appraisement,
stay or exemption laws of any state, now in force or hereafter to be passed.
If Credential Leasing Corporation has not received the full amount of any monthly
payment by the end ofthe fifth day of the month, we will pay a late charge to Credential Leasing
Corporation. The amount ofthe charge will be five (5%) percent of the overdue payment. We
will pay this charge promptly but only once on each late payment.
