DECREE OF THE REGISTER OF WILLS
AND NOW, this 30th day of September, 2008, upon consideration of the Petition for Grant of
Letters filed by Chirstina M. Chaback, for the above decedent and the instrument offered for probate as
the Last Will and Testament, which is dated October 5, 1994, the Register of Wills having given
consideration thereto, has made an official determination regarding the Last Will and Testament and
renders the following decision:
IT IS DECREED that the first three pages of the document and the page containing the
notarized acknowledgement of the testator are admitted to probate. The last page of the document titled
"Addendum to Last Will and Testament of: Joephine N. Grillo" is not admitted to probate as it is
unsigned and appears after the signature of the testator.
IT IS FURTHER DECREED that Letters Testamentary shall be issued to Christine M.
Chaback. Christina M. Chaback shall have all the rights and duties of a fiduciary under the laws of
Pennsylvania and shall proceed with the administration of this estate according to law.
