DECREE
AND NOW, ~~%~/2004, upon consideration of the petition
of Melody J. Moore on behalf of her daughter, Ashley Rose Moore, it
is hereby ordered and decreed that Melody J. Moore is appointed
guardian of the estate of Ashley Rose Moore, a minor.
No security will be required to be entered by the guardian.

PETITION FOR APPOINTMENT OF A GUARDIAN
OF THE ESTATE OF A MINOR UNDER THE AGE OF 14 YEARS
PURSUANT TO 20 PA. C.S. SECTION 5111(a)
TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
The Petition of Melody J. Moore respectfully represents:
1. Petitioner is the mother of Ashley Ro~:~!~Moo~,
child, who resides at 2112 Wentworth Drive, Camp Hill, Q~mber!and
County, Pennsylvania.
2. Ashley Rose Moore, the minor, is 12 years,of, ad, born., on
May 2, 1991, and resides with her mother at 2112 WentwoC~th Dr~-~e,
Camp Hill, Cumberland County, Pennsylvania.
3. The minor child is the child of Barry Lee Moore, who died
on December 29, 2003, and Melody J. Moore.
4. It is necessary that a guardian of the estate be
appointed for the minor because the minor has an interest in the
estate, including IRA proceeds, of her father.
5. No other guardian has been appointed for the minor's
estate.
6. The proposed guardian, Melody J. Moore, is 48 years of
age, born April 7, 1956, and resides at 2112 Wentworth Drive, Camp
Hill, Cumberland County, Pennsylvania.
7. The proposed guardian is the mother of the minor and has
no interest adverse to the minor. The consent of the proposed
guardian is attached as Exhibit A.
The minor has resided with petitioner throughout her
life.

The minor has a 1/2 interest in the estate and IRA of her
father, Barry Lee Moore, valued at less than $50,000.00, the IRA
proceeds, $1,136.82, are being held by Phoenix Investment Partners.
9. The estimated annual gross income from the estate and IRA
proceeds will be minimal. The minor will also receive a monthly
Social Security Survival benefit.
WHEREFORE, petitioner requests that this Court appoint Melody
J. Moore as guardian of the estate of her daughter, Ashley Rose
Moore, a minor.

CONSENT OF PROPOSED GUARDIAN
I, Melody J. Moore, hereby state that:
I hereby consent to act as the guardian of the estate of my
daughter, Ashley Rose Moore.
I reside at 2112 Wentworth Drive, Camp Hill, Cumberland
County, Pennsylvania.
I am a citizen of the United states and can speak, read and
write the English language.
I am not the fiduciary or an officer or employee of any
corporate fiduciary of an estate in which the minor has an interest
or the surety or an officer or an employee of the corporate surety
of such a fiduciary.
I have no interest adverse to the minor.
It is not my present intention to apply for an allowance for
the support or education of my daughter during her minority. I
recognize that I will require court approval before such an
allowance can be authorized.
