PETITION TO CHANGE NAME
TO THE HONORABLE, THE JUDGES OF SAID COURT:
AND NOW, comes the Petitioner, Klm Joanne Watson, by and through her attorney,
Mark F. Bayley, Esquire of Rominger, Bayley and Whare Law Offices, and respectfully
represents as follows:
1. Petitioner is Kim Joanne Watson, an adult individual, who resides at t2 Flagstone
Drive, Carlisle, Cumberland County, Pennsylvania.
2. Petitioner requests her name to be changed to Kim Joanne Welton because her
divorce has been finalized and she desires for more appropriate indication of
family connection by adopting her step-father's surname, Welton, in substitution
for her married surname.
3. Petitioner is 34 years of age, having been bom on April 19, 1969, at Jacksonville,
Duval County, Florida.
4. Petitioner's father's full legal name is William Harrison Hodges, Jr. Petitioner's
mother's full legal name is JoAnne Welton. and her maiden name is JoAnne
Serafini.
5. Petitioner has been a resident of Pem~sylvania since 1972. Prior to that, she
residedin Jacksonville, Florida.

Petitioner was married to James Watson, II1; she is now divorced. Petitioner has
no children.
Petitioner's original name, which appears on her birth certificate, was Kim Joanne
Hodges. Petitioner's married surname is Klm Joanne Watson.
Petitioner's occupation is Registered Nursing. She is currently employed at Holy
Spirit Hospital in Camp Hill, Pennsylvania. During the past 5 years, Petitioner
has worked for Holy Spirit Hospital from 2002 to the present, and Lock Haven
Hospital in Lock Haven, Pennsylvania, from 2000-2002.
Petitioner does not own and operate a business.
Petitioner is a registered nurse.
Petitioner has graduated from the following schools:
Diploma from Bucktail High School in Renovo, PA
Diploma from Geisinger Nuring School in Danville, PA
Petitioner has never been convicted of a crime.
Petitioner has never been adjudicated bankrupt.
Petitioner has no fraudulent purpose in changing her surname, having no creditors
(other than current accounts, all of which she intends to honor) nor judgments,
decrees or other process outstanding against her.
WHEREFORE, pursuant to 42 Pa.C.S. {}702, your Petitioner respectfully requests this
Honorable Court to order her name ofKim Joanne Watson to be changed to a new full name of
Kim Joanne Welton.
