4) The facts upon which this claim is based:
This claim is based on medical services which were provided by Holy Spirit Hospital for
the dates above listed. Bills were issued to the decedent for these services. We note that the
final two Hospital admissions occurred within six (6) months of the decedent's death and,
therefore, qualify for priority treatment under the Probate Estates and Fiduciaries Code 20
Pa.C.S.A. § 3392 which gives payment priority for the costs of medicines, medical, nursing and
hospital care furnished to the decedent within six (6) months of her death.
5) Decedent's Address: 2535 Rollo Court, South Middleton Township, Mechanicsburg,
Cumberland County, PA 17055.
6) Date of Death: 07/20/2007
7) The claim arose prior to the death of the Decedent.
