THE UNDERSIGNED, Alan Kotz, a/k/a Allen Kotz, does hereby swear and affirm as
follows:
I am an adult individual residing at 1512 Hemlock Farms, Hawley, PA 18428.
2. I was the brother-in-law of the late Eleanor Haselhuhn, Deceased, who died on
October 13, 2008.
3. Article TENTH of the Last Will and Testament of Eleanor Haselhuhn provides
for three (3) co-executors in the event that Eleanor Haselhuhn's husband is unable to serve.
4. Eleanor Haselhuhn's husband, Donald H. Haselhuhn, died in 1994.
5. I am one of the three named Co-Executors of the Estate of Eleanor Haselhuhn,
Deceased (the "Estate") in place of Donald H. Haselhuhn, Deceased.
6. The remaining two named Co-Executors are Heath L. Allen, who died July 29,
2008, and Wachovia Bank, successor to Hamilton Bank.
7. Gary E. French has been designated to replace Heath L. Allen as a Co-Executor
per terms of the Will.
8. A Petition for Grant of Letters was filed by me and Gary E. French on
October 30, 2008, at a time when I anticipated that Wachovia Bank would renounce its right to
serve as co-executor.

9. I have since been advised that Wachovia Bank will serve as a co-executor of the

Register of Wills on October 30, 2008, should be amended to include a request for grant of
letters to Wachovia Bank as a third co-executor for the Estate of Eleanor Haselhuhn, Deceased.
