Anna S. Pavlik, your Petitioner, files this Petition for Settlemerrt of a Small Estate ufl2ler
the provisions of Section 3102 of the Probate, Estates and Fiduciaries Code and in support
thereof avers that:
(1) Your Petitioner, Anna S. Pavlik, is a competent adult residing at 1007 Harriet
Street, Carlisle, Pennsylvania 17013, and is the surviving spouse of the above
decedent.
(2) Peter Pavlik, spouse of the Petitioner, died on May 17, 2008, at the age of 94
years, but prior thereto lived and was domiciled at 1007 Harriet Street, Carlisle,
Pennsylvania, Cumberland County, Pennsylvania. He died with a Will and no
letters Testamentary have been issued.
(3) Peter Pavlik had no probate estate when he died other than the following:
74 Shares of Prudential Financial, Inc. stock valued at $2,375.40.
(4) Your Petitioner avers that there are no creditors of the decedent and no claims
unpaid known to your Petitioner.
(5) Your Petitioner avers that the stock will be transferred to the surviving spouse
according to Paragraph 3 of the Last Will and Testament of the decedent.
WHEREFORE, your Petitioner respectfully requests that an Order be made authorizing
Petitioner to transfer all stock held with Prudential Financial, Inc., pursuant to Section 3102 of
the Probate, Estates and Fiduciaries Code.
