PETITION FOR CHANGE OF NAME
NOW comes the petitioner, ALEXANDER LAURENCE GAYER, by his attorney, Harold S. Irwin,
III, Esquire, and presents this petition for change of name, representing as follows:
1. Petitioner is ALEXANDER LAURENCE GAYER, an adult individual residing at 203 North
Prince Street, Shippensburg, Cumberland County, Pennsylvania 17257.
2. Petitioner's father's name was voluntarily changed from Laurence H. Gayer to Laurence
H. Geier some years back. At that time, petitioner, though a minor, did not want to change his
name.
3. Now that petitioner is an adult, he desires to change his last name from "Gayer" to
"Geier' so that he has the same last name as his father.
4. There are no judgments or decrees of like character of record against the petitioner.
WHEREFORE
petitioner requests y ur Honorable Court, pursuant to Act of December 16,
1982, P.L. 1309, No. 295, Section 2, 54 Pa. Cons. Stat. Section 701, et seq., to do the
following:
A. Fix a hearing on this petition; and
B. Enter a decree changing the name of petitioner from Alexander Laurence Gayer
to Alexander Laurence Geier.

has applied for a name change, has been searched in the files of the Pennsylvania State Police, Criminal Records and
Identification Division Central Repository, and:

( ) This person is subject to 18 PA C.S. Chapter 91, and the name change has been noted on the person's

( ) A criminal record was revealed containing felony convictions. IN ACCORDANCE WITH THE NAME
CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE WHEN A NAME
CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The Pennsylvania State
Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S., Chapter 91 (relating to criminal history record information).

( ) A criminal record was revealed that might prohibit a name change. Because the subsection(s) of the

there is no legal objection to the granting of the prayer of the petitioner,
