The Petition of JULIE MAY GODA, respectfully represents as follows: --'
1. Petitioner, Julie May Goda, resides at 1280 S. Mountain Road, Dillsburg, Pennsylvania
17019.
2. Respondent is the Estate of William James Allen Kimmel, who was deceased September 2,
2007. An estate was raised in Cumberland County having estate number 2007-00861 with Jean L.
Kimmel appointed as Administrator.
3. Petitioner, states that William James Allen Kimmel is the father of her child, Jordin Elizabeth
Gelbaugh, born April 26, 2008. An original of the child's Birth Certificate is attached hereto as
Exhibit "A". The certificate of birth does not list the Father's name.
4. Petitioner applied for Social Security benefits for the child upon the father's demise but was
subsequently denied since his name was not listed on the Birth Certificate.
5. The Social Security Administration office requests that the Petitioner have paternity
confirmed by Court Order.
6. The Petitioner and the deceased's parents obtained a DNA test which confirmed paternity,

STIPULATION OF PATERNITY
AND NOW, this / ~ day of ~ F f$n-.. ~r' , 2008, it is hereby AGREED
between the undersigned parties that the results of the DNA Test taken by DNA Diagnostics Center
shall be accepted as proof of paternity that William James Allen Kimmel is the father of Jordin
Elizabeth Gelbaugh.
AND FURTHER, the parties agree to have this Court confirm paternity with the respect that
William James Allen Kimmel is the late father of Jordin Elizabeth Gelbaugh.
