PETITION TO REQUEST PERMISSION TO PAY
GIFT TO MINOR INTO RESTRICTED ACCOUNT
AND NOW, comes the executrix, Donna K. McKeehan, by and through her
attorney, Michael A. Scherer, Esquire, and respectfully represents that:
1. The decedent, Lois J. Downes, died testate on March 6, 2r~D8~ `-
2. Donna K. McKeehan was appointed executrix of the estate by~~~he ~>

Register of Wills of Cumberland County on March 25, 2008. ~ -
3. The decedent left her estate in four equal shares to her children. `::~ _
4. One of the decedent's children, Charles Cohick, predeceased the ,..
decedent.
5. Charles Cohick died the father of five living children, including Adam
Cohick.
6. Adam Cohick is a minor, having been born on January 25, 1991.
7. Adam Cohick has an UGMA account at Ameriprise Financial, being
account number ----------6002.
8. The account representative at Ameriprise Financial is Scott Bechtold, who
has an office at 37 South Hanover Street, Carlisle, Pennsylvania.
9. Undersigned counsel has contacted Mr. Bechtold and has been advised
that were the Court to Order the funds payable from the decedent's estate to
Ameriprise Financial, the funds could be added to the minor's existing account and the
account could be marked that no withdraw is permitted until the minor's eighteenth I'
birthday.

WHEREFORE, the executrix respectfully requests that the Court issue an Order
that the funds payable to the minor from this estate be added to the minor's existing
account and the account be marked that no withdraw is permitted until the minor's
eighteenth birthday.
