DECREE
AND NOW, this day of ~ ~_~ 2008, after
a~nsideration of the Petition, the attached Exhibits and the parties' Stipulation, it appears that:
1. Julie May Goda is the natural mother of Jordin Elizabeth Gelbaugh.
2. William James Allen Kimmel is the natural father of Jordin Elizabeth Gelbaugh.
3. At the time of birth of the minor on the 26`h day of April, 2008, the father William James Allen
Kimmel was deceased.
NOW THEREFORE, it is ordered and decreed that the Chief, Division of Vital Statistics, and
State Registrar of the Department of Health of the Commonwealth of Pennsylvania shall amend birth
registration No. 048700-2008 so that the name of the father of child shall be added to the registration
and state that William James Allen Kimmel is the father.
